K-12 Standard Documents
As part of the implementation of Senate Bill 164 (SB164), the Utah Division of Facilities Construction and Management (DFCM) is developing standardized forms, templates, guidelines, and procedures to support consistent and efficient oversight of K–12 public school construction projects.
This page serves as a centralized resource for Local Education Agencies (LEAs), contractors, design professionals, and other stakeholders to access documents related to project planning, procurement, construction delivery, permitting, approvals, and regulatory compliance.
DFCM will continue to update this page as new documents become available and existing materials are revised to reflect current policies, procedures, and statutory requirements. Stakeholders are encouraged to check regularly to ensure they are using the most current versions of all applicable documents.
Pay Application Template
DFCM has developed a standardized Pay Application Template for Local Education Agencies (LEAs) to use when managing construction project payments. The template provides a consistent format for documenting and reviewing payment requests while allowing LEAs to customize the document with their own branding, logos, and agency-specific information.
