K-12 Standard Documents
As part of implementing Senate Bill 164 (SB164), the Utah Division of Facilities Construction and Management (DFCM) is developing an LEA resource manual detailing new processes and procedures to support consistent and efficient oversight of K–12 public school construction projects. When posted, the resource manual will be available on this page.
Additionally, this page provides Local Education Agencies (LEAs) with access to DFCM contract templates, solicitation templates, and other construction project management reference documents available for use at their discretion.
DFCM will continue to update this page as new documents become available. Stakeholders should check regularly to ensure they access the most current document versions.
EDMS Building Permit Application for Public School Construction
This fillable form is required for permitting public school construction projects. All projects requiring a building permit must go through the EDMS plan review process, which this form initiates. The School District Construction Authority (SDCA) or project architect/engineer on behalf of the school district should download, complete, and submit this form to [email protected].
Pay Application Template
DFCM has developed a standardized Pay Application Template for Local Education Agencies (LEAs) use. The template provides a consistent format for documenting and reviewing payment requests while allowing LEAs to customize the document with their own branding, logos, and agency-specific information. Using this document for each construction project will fulfill the LEAs' monthly reporting requirement.
